ONLINE CHECK-IN:

1.  We've made it easier for you to now CHECK-IN 24 hours prior to your appointment.  This form can be completed on any device.

ARRIVAL TO YOUR VISIT:

​

1.  Once you arrive to your appointment please send us a text message to 972-210-0452 saying "Here"

2.  Wait in your car until you receive a message to come in.

3.  ALL persons over the age 2 are REQUIRED to wear a mask entering and while in the building.  

4.  Wait for further instructions prior to entering the building. 

5.  ALL persons entering the building must go through a temperature check.

AFTER YOUR VISIT:

1.  Log on to the patient portal to download or review the visit notes.
